Contract | Days (5x8 Hours, 8:30–17:00) | Onsite

Key Responsibilities

Provide skilled physical therapy services to home health patients across a northern territory including surrounding communities
Visit up to 6 patients per day with a standard productivity expectation of 30 points per day
Complete documentation using EPIC Remote Client for Home Health and Rover phone application; OASIS completion is required
Manage daily patient visits with travel across a service area; occasional maximum drive radius of approximately 60 miles (60–90 minutes total drive time) per day
Maintain professional business casual or scrub attire during all patient visits

Required Qualifications

Active Oregon Physical Therapist license (compact license must be in hand at time of submission)
Minimum of 1 year of skilled home health experience required
Basic Cardiac Life Support (BCLS) certification required
Primary COVID-19 vaccination series plus one booster required
Must have reliable personal transportation
Two professional references covering at least 1 year of experience within the last 3 years required prior to start
Local candidates are welcome; candidates within a 50–75 mile radius are considered local and accepted

Preferred Experience / Skills

Familiarity with OASIS documentation in a home health setting
Experience with EPIC electronic health records
Comfort with independent scheduling and self-directed patient management in a community-based environment
