Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite

Key Responsibilities

• Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
• Manage a caseload of up to 6 patients per day, maintaining standard productivity expectations
• Complete OASIS assessments as required for home health documentation
• Chart using EPIC Remote Client for Home Health and a mobile phone application
• Travel independently to patient homes; occasional daily drive radius of up to approximately 60 miles

Required Qualifications

• Active Physical Therapist license valid in Oregon (compact license must be in hand at time of submission; pending licenses considered)
• Minimum of 1 year of skilled home health experience required
• Basic Cardiac Life Support (BCLS) certification required
• OASIS proficiency required
• Must have reliable personal transportation
• COVID-19 primary vaccination series plus one booster required
• Locals accepted; candidates within a 50–75 mile radius of the facility are considered local and are welcome to apply

Preferred Experience / Skills

• Experience with EPIC electronic health records platform
• Familiarity with home health productivity standards and point-based documentation systems
• Ability to manage an independent schedule with minimal supervision in a community-based setting
