About the Position

Job Title

Physical Therapist (PT) – Home Health

Location

Dickinson, ND

Schedule / Job Type

Contract | Days | Monday through Friday, 8:00 AM – 5:00 PM | No weekends or holidays

Key Responsibilities

Conduct start of care assessments, recertifications, resumption of care, and regular patient visits in a home health setting
Manage a typical caseload of 5–6 regular visits per day, or 3 additional visits on admission days
Maintain a daily point target of 6.5 points per day
Perform occasional safety evaluations or one-time teaching visits in hospice situations as needed
Document patient care accurately and in compliance with home health standards, including OASIS
Operate independently in the field while utilizing available clinical support resources

Required Qualifications

Active North Dakota Physical Therapy License
Current BLS certification
Valid Driver's License and active Auto Insurance
Ability to travel to patient homes within the service area

Preferred Experience / Skills

Prior home health experience strongly preferred
OASIS training and experience preferred
Comfort working independently in the field with remote clinical support
